1. 귀사의 발전을 기원합니다.

2. 본인은 재직 기간 중 귀사에 재직하였습니다.

3. 재직 기간 중 본인은 연장·야간·휴일근로를 수행하였으나, 근로기준법 제56조에 따른 가산수당이
   약정된 포괄임금 금액을 초과하여 발생하였음에도 차액이 지급되지 않았습니다.
   이에 따라 본인에게 미지급된 임금액은 금 0원입니다.

4. 본인은 포괄임금제 약정의 법적 유효성에 대한 대법원 판례(대법원 2020다219928, 2024.2.8 선고
   2018다206899 등)를 근거로 위 금액의 지급을 귀사에 청구합니다.

5. 본 통지문 수령 후 14일 이내에 위 금액을 지급하여 주시기 바라며,
   기한 내에 지급이 이루어지지 않을 경우 고용노동부 진정 및 민사 청구 등 법적 조치를
   취할 수 있음을 알려드립니다.

   - 급여명세서 사본 (해당 기간)
   - 근로시간 증빙 자료 (근태 기록·업무 기록 등)
